This service is ideal for optometrists, surgical assists, and locums who don't have their own accredited submitter. Fax forms are available for download here. Select the daysheet form that best suits your needs. If you need to get a new business arrangement number to use with us as your accredited submitter, download form 913 and send it to Alberta Health & Wellness after completing it. If you have an exisitng business arrangement number that you want to assign for use with us then download form 2096 and fax or mail it to us. We will complete it and forward it to Alberta Health & Wellness. |
